Because scar tissue is made of fibers, not skin cells, it does not have hairs, sweat glands or blood vessels and generally has a pale or pink shiny appearance.

FUT Scars

FUT hair transplants often leave a long, visible linear scar across the donor area, typically at the back of the head. These scars can widen over time, leading to a noticeable bald strip that forces many to keep their hair longer to hide the scarring. Unfortunately, even with longer hair, these strip scars can remain visible. Additionally, the direction of hair growth above and below the scar often changes, causing the hair on the back of the head to sit unevenly and making shorter hairstyles unsightly.

Several factors contribute to more prominent FUT scars, including:

  • Improper incision length (either too short or too long) resulting in scars that are too wide or widen over time
  • A tight scalp or poor surgical technique

Combining scalp micropigmentation with a hair transplant is a great solution for dealing with a surgery scar. SMP can effectively disguise the appearance of the strip  where hair doesn’t grow, creating the illusion of fuller, more even coverage – allowing for slightly more freedom around hair styles.

FUE Scars

While most transplants these days use the FUE method, FUE hair transplants are they do generally still leave scarring in the donor area. Although the FUE process is less invasive than FUT, it is still a surgical procedure and involves extracting individual hair follicles using a small punch (typically 0.7mm–0.9mm), which creates tiny scars. When viewed collectively, these scars can give the donor area a “moth-eaten” or patchy appearance, especially if too many hairs are extracted from one area.

Prominent FUE scars can result from:

  • Over-extraction of hair follicles from the same area (e.g., 2,000–5,000 punches in one region)
  • Poor distribution of follicle extraction, leading to uneven, patchy donor areas
